This registration form is for the following:
    Cherub Choir - 1st and 2nd graders
    St. Lucy Choir - 3rd to 6th graders (used to be Children's Choir)
    Cantores Choir - 7th and 8th graders
    St. Maria Goretti Handbell Choir - 3rd to 8th grade graders

The Registration process is as follows:
    1. Complete one form for each person.
    2. Fields with an * are required.

All parents (and grandparents, etc.) who want to serve as volunteers in direct contact with minor children must be screened
and cleared under the Diocese of Dallas Safe Environment Program. Screening is a multi-step process. Please see the church

REGISTRATION FEE (includes Snack Fee, Activity Fee, T-Shirt cost, etc.) for the CHOIR YEAR:
CHERUB CHOIR (Grade 1 & 2) - $30 for each child; For 2 siblings from the same family in the same CHOIR, the cost is $50.
St. Lucy CHOIR (Grade 3 - 6) - $30 for each child; For 2 siblings from the same family in the same CHOIR, the cost is $50.
ST. MARIA GORETTI CHILDREN'S HANDBELL CHOIR (Grade 3 - 8) - $30 for each child; For 2 siblings from the same family in the same CHOIR, the cost is $50.
CANTORES CHOIR (Grade 7 & 8) - $30 for each child; For 2 siblings from the same family in the same CHOIR, the cost is $50.

All CHECKS should be made payable to "ST. PATRICK CATHOLIC CHURCH'" with your child's name(s) clearly printed on the memo line.
If you are registering your child for more than one choir, please include a separate check for each different choir's registration fee.
You may leave an envelope with your child's choir registration fee(s) at the CHURCH OFFICE. Please address it to "MUSIC."
If the Church Office is closed, you may use the Night Drop Mail Slot located to the far left of the church office's front doors.
Although payment via check is preferred, you may leave a sealed ENVELOPE addressed to MUSIC and your child's name with your EXACT amount in CASH if necessary.

Here is a description of the volunteer options.

CHOIR REHEARSAL ASSISTANTS (take attendance, give out nametags, escort to restrooms, serve snacks, etc., 1.5 hours/once a month/all semester/all year?

CHOIR CARPOOL TEAM (come 10 minutes before end of rehearsal, set out 3 traffic cones, help children get into cars, take up name tags & cones 30 minutes for a month/semester))

CHOIR MASS PARENT ASSISTANTS (give out nametags, music sheets, escort to restroom, help seat children in church choir loft, etc. 30 minutes before Mass time; sit with children during mass; help with clean-up 10 minutes after mass)

CHRISTMAS PAGEANT REHEARSAL ASSISTANTS. (help with props, scenery, chair set-up, microphones, at practices in December and Christmas Eve 4:00 Mass)

CHRISTMAS PAGEANT COSTUME COMMITTEE member (measure and fit costumes for singers during rehearsals in December; check-in costumes after Christmas Eve 4:00 Mass)

CHRISTMAS PARTY COMMITTEE Member (purchase food with Choir ACTIVITY money/serve/clean-up PARTY - December 23, 12:00 pm)

END-of-YEAR PARTY COMMITTEE Member (purchase food with Choir ACTIVITY money/serve/clean-up PARTY - To Be Determined - in May)

SNACK COMMITTEE MEMBER (purchase snacks w/choir funds weekly or monthly & deliver to choir room)

CHOIR COMMUNICATOR PARENT (create choir email list; send weekly Email rehearsal reminders and sent special ones from Mrs. Fruth)

MAKE CHOIR Nametags/CARPOOL TAGS with materials bought for you

ON-CALL HELP Team MEMBER (Help with Projects as needed--help set up instruments for Cherub Choir Reward Spending Day at 2-3 rehearsals during the year; stuff Easter Eggs with candy for our Easter Egg Hunt, etc.)

CHOIR PARENT VOLUNTEER CO-ORDINATOR (update Google Docs Database of Parent Volunteers & Make Sure Above Volunteers are Assigned to Jobs - recruit Snack Chairman from Snack Volunteers; Rehearsal Asst. Co-ordinator; as each area is outlined above; assign dates for jobs/send reminders)

CHERUB CHOIR TREASURER (collect money from each chorister's parent(s); deposit checks/money to CHERUB CHOIR ACTIVITY FUND with Anita Saunders in Church OFFICE, request funds/reimbursements for choir activities, keep records)

CHRISTMAS CONCERT Committee (Final Date TBA mid-December - Advertising, Program Help, Set-UP, Clean-up, etc.)
